Abstract:Inferior alveolar nerve block is the most commonly prefer anesthetic procedure for mandibular dental treatments. In the inferior alveolar nerve block injection, the target area where the local anesthetic agent is to be stored is the mandibular foramen and its surroundings. The location of mandibular foramen varies considerably due to ethnic and racial differences in jaw growth patterns and different age groups. Failure of inferior alveolar nerve block application may lead to repeated injections and the occurrence of cooperation problems in children. In addition, this can lead to administration of an anesthetic solution in amounts exceeding the maximum toxic level. The aim of this study was to evaluate the relationship of mandibular foramen used as a clinical guide in inferior alveolar nerve block injection with the anterior border of mandibular ramus and the occlusal plane in children of different age groups. Three hundred and sixty panoramic radiography of children between 3 and 13 years old was selected from the database of Gazi University Faculty of Dentistry and divided into six groups as 3-4 years, 5-7 years, 7-9 years, 9-11 years, 11-12 years and 12-13 years. The radiographs were analyzed digitally. Anatomical landmarks were marked and linear measurements were noted from the most anterosuperior point of mandibular canal to the occlusal plane, and to the deepest point on anterior border of mandibular ramus. The data obtained was tabulated and subjected to statistical analysis. The results of this study suggest that the mandibular foramen is below the occlusal plane in 3–4 year-old children (average 0,27 mm), above the occlusal plane in 5–7 years (average 2,12 mm), 7–9 years (average 1,74 mm), 9–12 years (average 2,32 mm), 11–12 years (average 2,47 mm), and 12–13 years (average 2,97 mm). When the parameter of the distance of the mandibular foramen to the anterior border of the ramus is examined, mean values of 18,1 mm, 18,59 mm, 19,49 mm, 19,54 mm, 20,32 mm and 19,74 mm were reached for 3-4, 5-7, 7-9, 9-11, 11-12 and 12-13 age group children respectively. As a result, an increase was observed in the distance of the mandibular foramen to the occlusal plane and the anterior border of the ramus due to growth and development.
