Abstract:Hypothyroidism is a common disease and is known to cause hypertension,especially diastolic hypertension. Nondippers are individuals with absence of anticipated nocturnal decrease in blood pressure and increased incidence of target organ damage and a more incidence of essential hypertension. It has been shown that hypothyroidism can cause nondipper blood pressure pattern. We aimed to investigate the effect of thyroid hormone replacement therapy in overt and subclinical hypothyroidism on the blood pressure pattern via 24-hour ambulatory blood pressure monitoring (ABPM). 29 normotensive patients with overt and subclinical hypothyroidism, that had been diagnosed in 2008, were evaluated. All participants underwent 24-h ABPM evaluation before and after thyroid hormone replacement therapy. Before the second ABPM evaluation all patients were euthyroid. The analysis of ABPM datas were compared. The change of fasting serum LDL and triglyceride levels were not statistically significant. Hemoglobin level was significantly higher after therapy. Đn our whole study group and in the patients who were older than 40 years old, day time and 24 hour systolic and mean blood pressure were significantly higher in patients after they received thyroid hormone replacement therapy compared with the datas measured before they received therapy. After therapy the decrease of non-dippers number was statistically significant just in the group which included patients younger than 40 years old (p:0,046). The number of diastolic non-dipper individuals decreased from 5 to 2 with thyroid replacement therapy in patients who were younger than 40 years old and had subclinical hypothyroidism before treatment (p:0,083). Consequently, although limited number of participants we believe that thyroid hormone replacement therapy can improve dipping profile, especially in the young population.
