Abstract:In recent years, the rate of successful pregnancies through assisted reproductive treatments for infertility has increased gradually. However, techniques applied in order to stimulate ovarian follicles lead to some complications. One of the worst and even life- threatening complication of these techniques is OHSS. Three mechanisms are suggested to be underlying in the pathogenesis of OHSS. The first mechanism is the fluid escape from intravascular space to extravascular space and body cavities due to increased VEGF sensitivity. Eventually, hypotension and shock develops due to intravascular volume reduction. The second mechanism is inflammatory process due to the increase of inflammatory mediators. And the third mechanism is excessive fluid escape to serous body cavities such as peritoneal, pleural cavities as a result of Renin-Angiotensin-Aldosterone system overactivation. Melatonin is a lipophilic hormone which can pass blood-brain barrier readily. Melatonin is found not only in the systemic circulation but also within the follicular fluid and has an important role in the maturation of the follicles. It may be considered that melatonin may be a very beneficial agent in the treatment of OHSS owing to its antioxidant, anti- inflammatory effects and regulatory role in VEGF expression. In the present study, 22 days old female Wistar Albino rats were randomly divided into 4 groups, each of which containing 7 rats and an experimental OHSS was induced in one these groups. All animals in all groups were sacrifised at the end of the experiment and ovaries were harvested. Macroscopic evaluation of ovaries was fallowed by histomorpometric analysis. Additionally, blood levels of E2 (estradiol), and VEGF and IL-6 levels in peritoneal fluid were measured through ELISA.
