Abstract:In this study, sharing the expertise of Gazi University School of Medicine Epilepsy Center is aimed. 383 patients who underwent epilepsy surgery between January 2002 and June 2017 were studied. Cases are grouped; Temporal lobectomy (237), VNS, (138), and hemispherotomy (8). Clinical, semiological, neurophysiological and neuroradiological characteristics, mortality, morbidity and complications developed after surgery were screened and seizure control rates were revealed. 174 patients in the TLE group, Engel Class I 84.5% (147), Engel Class II 9.2% (16), Engel Class III 4% (7), and Engel Class IV 2.3% (4). There was no surgical mortality. Complications were found in 11.4% of patients and complications needed surgery were 5% of patients. In 79 patients in the VNS group, according to the McHugh classification, Class I 8.9% (7), Class II 24.1% (19), Class III 36.7% (29), Class IV 13.9% (11) and Class V 16.5% (13). Surgical mortality was not observed and 5 patients had late mortality. Surgical field infection was seen in 7 patients, system was removed in 5 patients, postoperative hematoma was developed in 4 patients, and 3 of them required surgical revision. In the hemispherotomy group, 6 patients (75%) classified as Class I, 1 patient (12.5%) Class II, and 1 patient (12.5%) Class III according to the Engel classification. There were no surgical mortalities, 1 patient underwent revision 136 due to CSF fistula and 1 patient underwent surgery due to epidural hematoma. Epilepsy surgery is a successful treatment for drug resistant epilepsy, and mortality, morbidity and complication rates are low.
