Project Abstract

ABSTRACT Malignant Pleural Mesothelioma (MPM) is an aggressive tumor derived from pleural mesothelial cells. The treatment approach in MPM, which has an mean survival time of 9-12 months in the literature, is multimodal treatment including surgery, chemotherapy and radiotherapy. Although the histopathological subtype is the most important factor in response to treatment, independently of this, the response to treatment may differ from patient to patient. This reveals the fact that factors other than histopathological subtyping also affect survival. There is a limited number of studies in the literature showing that parameters such as PD-L1, Ki-67 and TPS also have prognostic significance. It is necessary to reveal whether these parameters are a precise prognostic factor in MPM management with multicenter studies involving more patients. In this study, we aimed to analyze the prognostic significance of parameters such as inflammatory parameters, Ki-67, PD-L1 and TPS with an international, multicenter study. The data of patients who underwent pleurectomy/decortication for Malignant Pleural Mesothelioma between 2009-2021 in Gazi University Faculty of Medicine, Department of Thoracic Surgery, and UK Krems and KH Nord Wien Clinics, which are the other multinational branches of the study, in addition to general demographic characteristics, new promising factors such as inflammatory parameters, PD-L1, Ki-67, TPS and Fibrinogen were analyzed. When the data of our center were analyzed, it was found that PD-L1, Ki-67, hemoglobin and thrombocyte were factors that affected survival statistically. Whenever the results are analyzed again with the data obtained from other centers, more precise threshold values for these prognostic factors can be determined. In conclusion, when the data of other centers are obtained, the data will be re-analyzed and prognostic biomarkers such as PD-L1, Ki 67 will contribute to the literature with effective threshold values in order to predict the survival and the response to treatment in MPM patients.
