Impact of orthognathic surgery on oral health-related quality of life and patient satisfaction: a retrospective analysis of 65 patients

Objectives: To evaluate the impact of orthognathic surgery on oral health-related quality of life (OHRQoL) and patient satisfaction, and to analyze postoperative changes across functional, aesthetic, and psychosocial domains.
Materials and Methods: This retrospective questionnaire-based study included 65 patients who had previously undergone orthognathic surgery at Gazi University Faculty of Dentistry. Participants were between 18 and 40 years of age and had completed at least one month of postoperative follow-up.
Oral health-related quality of life was assessed using the validated Oral Health Impact Profile-14 (OHIP-14) and the Orthognathic Quality of Life Questionnaire (OQLQ). Patient satisfaction was evaluated using a structured satisfaction survey. Participants were asked to retrospectively assess both their preoperative and postoperative conditions.
Statistical analyses were performed using SPSS software (IBM Corp., version 27). Normality was assessed using the Shapiro–Wilk test. Paired samples t-test or Wilcoxon signed-rank test was applied as appropriate. Statistical significance was set at p < 0.05.
Results: A statistically significant improvement was observed in overall OHRQoL scores following orthognathic surgery (p < 0.001). Both OHIP-14 and OQLQ total scores demonstrated significant postoperative reductions, indicating improved quality of life.
All OQLQ domains—including oral function, facial aesthetics, social aspects, and awareness of dentofacial deformity—showed significant improvement. Patient satisfaction levels were high, with the majority of participants reporting that they would choose to undergo the procedure again and recommend it to others.
Subgroup analysis revealed comparable quality-of-life improvements across sexes, with no significant long-term differences in overall outcomes.
Conclusion: Orthognathic surgery significantly enhances oral health-related quality of life and achieves high patient satisfaction. Beyond functional correction, the procedure provides substantial psychosocial and aesthetic benefits. These findings support the importance of patient-centered outcome measures in evaluating orthognathic treatment success.
