Cerebrovascular disease is pathological damage to one or more blood vessels related to the brain. The disease causes significant effects on the country's economy and health budget with the resulting loss of labor or death. With the adoption of healthy lifestyle behaviors, continuous and comprehensive evaluations, and systematic nursing care, risk factors that can be changed in SVH can be eliminated, complications that may develop after the disease can be prevented, and the incidence of the disease can be reduced. This study aimed to present an example of a care plan according to the Nursing Model Based on Life Activities of a patient with SVH. It is thought that the care plan sample prepared for this purpose will contribute to improving the care and life quality of the patients with the use of the theories in care, as well as being a resource for nursing students in practice.
