Are We Safe for Bladder Outlet Obstruction and Underactive Bladder in Women Presenting Only WithStorage Symptoms?

Abstract: Objective: The aim of this study is to investigate the prevelance of voiding symptoms(VS) in women with urodinamically confirmed bladder outlet obstruction(BOO) or under active bladder(UAB) who presented with storage symptoms(SS). Methods: A total of 793 female patients aged 18–75 years, who presented only with lower urinary tract SS and had sufficient urological evaluations, were included in the study. BOO was diagnosed using the Solomon-Greenwell nomogram. VS presence detected in symptom investigation was examined in BOOdiagnosed patients. The projected isovolumetric pressure (PIP1) formula was used for UAB diagnosis. VS presence detected in symptom investigation was examined in UAB-diagnosed patients. Results: It was noted that 360 of the 793 patients included in the study had sufficient quality pressure-flow studies, and analyses were conducted on the data of these patients. It was observed that 78 (21.6%) of the patients had a bladder outlet obstruction in females (BOOIF) score >18. Among these patients, 41 (52.5%) had VS, while 37 (47.5%) did not. Among the 135 patients with VS, 41 (30.4%) had a BOOIF score >18, while among the 225 patients without VS,  37 (16.4%) had a BOOIF score >18. A statistically significant difference was noted between the two groups (p = 0.002). In 26 patients, a PIP1 value of <30 was detected. Among these patients, 13 (50%) had VS, while 13 (50%) did not. Among the 135 patients with VS, 13 (9.6%) had a PIP1 value <30, while among the 225 patients without VS, 13 (5.7%) had a PIP1 value <30. No statistically significant difference was found between the two groups (p = 0.172). Conclusion: In our study, a statistically significantly higher BOO was found in patients with concomitant SS-VS. However, the presence of VS accompanying SS did not make a statistically significant difference in the diagnosis of UAB.
