Heparin is commonly used during cardiopulmonary bypass for anticoagulation. However, despite its status as the gold standard, there are limitations. It is contraindicated in patients allergic to heparin or protamine, as well as those with heparin-induced thrombocytopenia. Bivalirudin, a direct thrombin inhibitor, has been reported as a safe and effective alternative in cases where heparin cannot be used due to these contraindications. Here, we present our experience using bivalirudin in a patient diagnosed with heparin resistance.
