Özet
Introduction: Severe mandibular ridge resorption may limit implant placement and compromise full-arch
rehabilitation. Autogenous bone grafting remains predictable for horizontal ridge augmentation. This re-
port describes full-arch fixed implant-supported rehabilitation following iliac crest grafting and a digital
prosthetic workflow.
Case Description: A 44-year-old female patient with no significant medical history presented for replace-
ment of an existing mandibular two-implant overdenture and maxillary fixed prosthetic rehabilitation. Fol-
lowing clinical and radiographic evaluation, conversion to a fixed implant-supported mandibular restora-
tion was planned. Due to insufficient buccolingual bone width in the posterior mandible, iliac crest bone
grafting was performed before implant placement. After graft healing, four mandibular implants were
placed and allowed to osseointegrate. Subsequent bone exposure was managed with a connective tissue
graft. After soft tissue healing, multi-unit abutments were selected, and the vertical dimension was estab-
lished with conventional wax rims. Implant and tooth positions were recorded using intraoral photogram-
metry. Impression accuracy was verified with a metal bar and PMMA provisional restoration, both demon-
strating passive fit. To accommodate mandibular flexure, the mandibular prosthesis was fabricated as two
separate segments divided between the central incisors. Definitive monolithic zirconia restorations were
delivered.
Discussion: This case demonstrates that successful fixed rehabilitation can be achieved in patients with
severe mandibular bone deficiency through staged surgical treatment, soft tissue management, and a dig-
ital workflow. Iliac crest grafting, implant therapy, and photogrammetry-assisted procedures facilitated
predictable functional, esthetic, and phonetic outcomes.
