The large need for organs in renal transplantation programs and the insufficience of living related donation lead us to evaluate every cadeveric organ available from marginal donors. Dual kidney transplantation is one way of using these marginal donors. We present a case of a five-year-old boy with brain death whose organs were harvested and both kidneys transplanted via an en bloc technique to a 27-year-old male patient. The kidneys functioned immediately after transplantation and the patient was discharged with a creatinine level of 0. 7 mg/dl.
