Vitreopapillary Interface Features May Contribute to NAION Pathogenesis

Özet

Purpose: To study the vitreopapillary interface in nonarteritic ischemic optic neuropathy (NAION).

Methods: The vitreopapillary interface was studied in 32 NAION eyes using spectal domain optical coherence tomography scans. Results were compared with 31 non?NAION eyes with small optic discs (Control I) and with 32 normal-sized optic discs (Control II).

Results: The rate of posterior vitreous detachment (PVD) over the macula was similar (NAION: 62.5%, Control I: 61.3% and Control II: 65.6%, P = .93), while posterior hyaloid remained attached over small discs at a significantly higher rate (NAION: 81.2%, Control I: 83.9% and Control II: 43.7%, P = .0005). A higher rate of strong focal vitreopapillary adhesions (tufts) on small discs compared to normal-sized discs was noted (NAION: 72.2%, Control I: 58.7% and Control II: 19.1%, P = .007). These tufts in eyes with NAION were gathered above the optic nerve vessels (NAION: 69% versus Control I: 3%, P = .00001). Conclusion: Small discs may have stronger vitreopapillary attachments. A close relationship of these tufts with optic nerve vessels may lead to a strong vitreal traction that may contort vessel wall and disrupt the blood flow in eyes with NAION.
