A 73-year-old male patient with prostate cancer underwent a staging Gallium-68 (Ga-68) prostate-specific membrane antigen (PSMA) positron emission tomography/magnetic resonance imaging (PET/MRI). Ga-68 PSMA PET/MRI revealed cervical and mediastinal lymph nodes, and a thyroid mass with moderately increased uptake, as well as extensive metastases of prostate cancer. The thyroid mass and lymph nodes were assessed suspicious for a second primary thyroid cancer due to their relatively lower radiotracer uptake. Pathological examination demonstrated multifocal medullary thyroid carcinoma (MTC) with multiple lymph node metastases. Our case highlights incidental detection of MTC on Ga-68 PSMA PET, likely related to PSMA expression in tumor-associated neovasculature. PSMA-targeted imaging and radioligand therapy could provide new diagnostic and therapeutic opportunities in patients with MTC.
