Mesane Kanserlerinde İntraoperatif Radyoterapi

ABSTRACT Organ-sparing approach is an alternative to radical surgery in the treatment of bladder car-

cinoma. However, applying high radiation doses to tumors does not always be possible without harm-

ing the surrounding normal tissues. Radiation dose can be given directly to the target by separating the

critical structures during surgery with the intraoperative radiotherapy (IORT) technique. Therefore, IORT

is included to be part of the organ-sparing approach. According to the studies in the literature, IORT is

feasible in the treatment of bladder carcinoma. However, there is still no consensus about the proper pa-

tient selection and radiotherapy dose. Also, IORT requires special applicators for different sites, an ex-

perienced team and cooperative working team in a multidisciplinary setting. Therefore, IORT does not

commonly use in the clinical practice yet.

ÖZET Mesane kanserinde organ koruyucu yaklaşım, cerrahi tedaviye alternatif oluşturmaktadır. Ancak,

lokal ileri evre hastalıkta çevre dokulara zarar vermeden tümör bölgesine yüksek doz vermek her zaman

mümkün olmamaktadır. İntraoperatif radyoterapi (IORT), cerrahi sırasında dozun en konformal şekilde

verilmesine olanak sağlamaktadır. Bu nedenle IORT’nin organ koruyucu yaklaşımda yer alabileceği dü-

şünülmüştür. Literatürde bu konuda yapılmış çalışmalar bunun uygulanabilir bir teknik olduğunu gös-

termiştir. Ancak, uygun hasta ve doz seçimi konusunda ortak bir kanı oluşmamıştır. Ayrıca, IORT uygun

ekipman, tecrübeli ekip ve multidisipliner çalışma gerektiren ayrıntılı bir işlemdir. Bu nedenle, günlük

pratikte yaygın olarak kullanılmamaktadır.
