Treatment Approach to a Discolored Tooth Due to Trauma

Introduction

The loss of vitality of the teeth due to trauma may cause discoloration of the teeth over time. In the rehabilitation of these teeth causing aesthetic problems, restorative approaches can be applied in addition to whitening treatment. In this case report, it is aimed to present the treatment approach applied to a male patient who applied to our clinic due to discoloration in tooth #11.

Case Report

In the anamnesis taken from the patient, it was learned that he was a professional football player and had trauma to his upper jaw 16 years ago. In the clinical examination, it was determined that there was a caries lesion on the mesial surface of tooth #11 and that the tooth was devital. In the radiographic examination, it was determined that the 1/3 coronal part of the root canal of the tooth could not be observed. After root canal treatment, it was decided to apply intracoronal bleaching treatment to the patient. After the patient was informed about the treatments to be performed, written consent was obtained.

During root canal treatment, the buccal wall of the root was perforated due to calcification of the root canal entrance. MTA was placed in the perforated area. At the second appointment, the main root canal was entered, enlarged and filled with gutta-percha. The cavity was then closed with temporary filling material.

At the third appointment, after the temporary filling material was removed, 2 mm thick flowable resin composite and bleaching material (Opalescence™ Endo, Ultradent Products, Inc.) were placed on the coronal entrance of the root canal and closed with temporary filling material. This application was applied until the desired tooth color was obtained. After the bleaching treatment, the tooth was restored with composite resin. The patient was asked to come for control after 3 months.

After 3 months, it was observed that the patient had no aesthetic or functional complaints in the restored teeth.

Conclusion

Restoring with resin composite after bleaching can meet the aesthetic demands of the patient in teeth discolored due to trauma.
