The clinical significance of the reverse distribution in 99m-Technetium sestamibi (99mTc-MIBI) myocardial perfusion scintigraphy is controversial. In clinical practice, it is often assumed as artefactual. This finding should be approached with caution. We report a case of a 72-year-old patient who exhibited reverse distribution in two-day stress/rest 99mTc-MIBI myocardial perfusion imaging protocol. The patient was diagnosed with non-ST elevation myocardial infarction on the day of rest imaging with scintigraphy. (c) 2024 Elsevier Masson SAS.
