Özet
Introduction: This case report aims to present the prosthetic rehabilitation of an edentulous maxilla and mandible using zygomatic and mandibular implants combined with a CAD/CAM fiber-reinforced composite framework, which may reduce stress on peri-implant bone due to its low elastic modulus and shock absorbing properties.
Case description: A completely edentulous patient with two zygomatic and two standard implants in the maxilla, and four standard implants in the mandible was treated for full-arch prosthetic rehabilitation. Following clinical and radiographic evaluation, open-tray impressions were taken for both arches. A full-arch hybrid prosthesis was planned and fabricated using a CAD-CAM milled fiber-reinforced composite framework. The framework was veneered with acrylic resin. After verifying passive fit, occlusion, and esthetics, the prosthesis was torqued to manufacturer-recommended values. The prosthesis demonstrated a passive fit on both arches, with satisfactory occlusion and esthetics. No biological or mechanical complications were observed during the follow-up period. The patient reported high satisfaction with function, comfort, and appearance.
Discussion: The use of a CAD/CAM fiber-reinforced composite framework in full-arch implant-supported rehabilitation, including zygomatic implants, appears to be a viable option offering favorable biomechanical behavior, passive fit, and satisfactory esthetic outcomes. This approach may reduce stress transmission to peri-implant bone due to its shock-absorbing properties and low elastic modulus.
