Özet
TRAVMA HİKAYESİ BULUNAN MAKSİLLER ANTERİOR DİŞTE TEDAVİ YAKLAŞIMI
Amaç:
Bu olgu raporunun amacı, komplike kron kırığına bağlı ileri derecede madde kaybı bulunan maksiller anterior dişin uygun tedavi ile estetik ve fonksiyonunun yeniden kazandırılmasını sunmaktır.
Olgu:
Sağ üst yan kesici dişinde (#12) kırık şikayeti ile Gazi Üniversitesi Diş Hekimliği Fakültesi Çocuk Diş Hekimliği Kliniği’ne başvuran 11 yaş 6 aylık hastanın anamnezinde alerjik astım hastalığı bulunduğu ve ilgili dişin yaklaşık 10 ay önce düşmeye bağlı travma sonucu kırıldığı öğrenildi. Klinik muayenede #12 numaralı dişte komplike kron kırığına bağlı ileri derecede koronal madde kaybı bulunduğu, kök kanalının açıkta olduğu ve dişin devital olduğu tespit edildi; perküsyon ve palpasyon testlerinin negatif olduğu ve mobilite bulunmadığı belirlendi. Radyografik değerlendirmede ilgili dişin kök gelişimini tamamlamış olduğu ve periapikal bölgede radyolusent lezyon bulunduğu belirlendi. Hastanın kliniğe ilk başvurduğu seansta #12 numaralı dişte kök kanal tedavisine başlandı ve 4 hafta süreyle kalsiyum hidroksit medikamanı uygulandı. İkinci seansta lateral kondenzasyon tekniği ile güta perka ve kök kanal dolum patı kullanılarak kanal dolumu gerçekleştirildi. Kök kanal tedavisinin tamamlanmasını takiben 1 hafta sonra fiber post uygulaması amacıyla post boşluğu hazırlandı. Ardından dual-cure rezin siman ile uygun çaptaki fiber post yerleştirildi. Fiber post uygulamasının ardından strip kron kullanılarak kompozit rezin ile restorasyonu tamamlandı. Hastanın 14 aylık klinik ve radyografik takibinde #12 numaralı dişte periapikal iyileşmenin gerçekleştiği ve uygulanan restoratif yaklaşım ile estetik ve fonksiyonel rehabilitasyonun sağlandığı gözlendi.
Sonuç:
Travmaya bağlı komplike kron kırığı bulunan ve ileri derecede madde kaybı olan anterior dişlerde kök kanal tedavisi sonrasında uygulanan fiber post destekli restorasyonlar, dişin fonksiyonel ve estetik rehabilitasyonunda başarılı bir tedavi seçeneği sunabilir.

TREATMENT APPROACH FOR A MAXILLARY ANTERIOR TOOTH WITH TRAUMA HISTORY
Objective:
The objective of this case report was to present the restoration of esthetics and function of a maxillary anterior tooth with severe substance loss due to a complicated crown fracture through appropriate treatment.
Case:
An 11-year-6-month-old patient presented to the Pediatric Dentistry Clinic of Gazi University Faculty of Dentistry with the complaint of a fractured upper right lateral incisor (#12). The patient’s medical history revealed allergic asthma, and it was learned that the involved tooth had fractured approximately 10 months earlier as a result of trauma caused by a fall. Clinical examination revealed severe coronal tooth structure loss due to a complicated crown fracture in tooth #12, with the root canal exposed and the tooth non-vital. Percussion and palpation tests were negative, and no mobility was detected. Radiographic evaluation showed that the tooth had completed root development and a radiolucent lesion was present in the periapical region.At the patient’s first visit, root canal treatment was initiated in tooth #12 and calcium hydroxide medication was applied for four weeks. At the second appointment, root canal obturation was performed using the lateral condensation technique with gutta-percha and root canal sealer. One week after the completion of root canal treatment, a post space was prepared for fiber post placement. Subsequently, a fiber post of appropriate diameter was placed using dual-cure resin cement. Following fiber post placement, the tooth was restored with composite resin using a strip crown.At the 14-month clinical and radiographic follow-up, periapical healing of tooth #12 was observed and esthetic and functional rehabilitation was achieved.
Conclusion:
Fiber post-supported restorations following root canal treatment may represent a successful treatment option for the functional and esthetic rehabilitation of anterior teeth with severe substance loss due to traumatic complicated crown fractures.
